Haltech wideband low voltage reading

i have the haltech wide band controler. (innovates with the haltech name)
anyways i am getting a very low voltage from my sensor.
i see at idle anywhere from 70 to 300 Mv and under acceloration i will see at most 2500 MV.
both are dangerously lean a/f ratios.
i know my car is not running lean, because i hooked up to my 4 gas and i have high ppm of HC so i know i am running super rich.
i started leaning my mix based on the 4 gas readings and that brought my wide band voltage up.
i am also getting the same voltage on my labscope so it is not in my software.
i have calibrated the sensor twice with the led and there are no error codes
heres how i have it hooked up
brown 0 to 5 v signal wire to purple shielded wire ecu side
and in my settings i have my o2 voltage to 0 to 5 volts and set to bosch wideband
one other question is any one having problems running there wideband sensors in the stock location due to heat issues

I'm not completely sure I understand as I haven't fooled with the Haltech/Innovative controller. But if you're trying to hook it up to the Haltech to read the linear 0-5V+ output from the sensor then your low voltage reading corresponds to the mixture being rich and not lean. If you're trying to use a narrow band output voltage to tune you're greatly under-utilizing the product.
