Check the injectors maybe, sounds like I had a similar issue and it was a bad primary injector.

BUMP for further investigation....

After 3-4 hours of badmouthing against Mazda designer I reached and removed fuel rail...

Rubber on primary rail are splitted and really toasted, rubber in secondary rail "hole" (on engine) are the same....

Primary injectors seems very dirty...

I have to look at injectors oring but i hope I have founded the problem.... (no problem on secondary injectors operating range)

Solved after a little time XD

One of primary injectors is totally broken (can't hold pressure and leak)

Bought 2 used injectors and cleaned them....now car run perfect
