The flanges rust over time and leaks appear due to the corrosion. The gasket in between breaks down and the more you drive, the more the gasket is compromised due to the bad flange. Other cases if a bad flange are a warped flange on a new pipe caused by improper torque application when drawing the flanges together. The flanges should be drawn together evenly, not cranked down on one side and then attempted to be cranked down on the other.
