Question Exedy Clutch Disk Failure - Seeking Clutch Advice

Greetings, (first post...take it easy on me...)
I have a 93 R1. Had to do the 5th gear synchro at about 95k and decided to replace the original clutch. Original clutch worked just fine for me, so replaced it with an Exedy (stage 1 I think - similar to OEM). At 103k, the disk blew up (rivets failed - and ripped itself to pieces). I'm dyno'd only at 260 hp and 225 lb-ft, but drive it like it's fun, and a bit of autocross. But, the same abuse that was applied to the failed new clutch, was applied to that old original clutch.
I believe the disk had a manufacturing flaw. The rivet/caps didn't have good penetration/bonding that seems clear after study. Wondering if anyone else has had a similar issue (especially on a near-stock car). I haven't got a response from Exedy yet (waiting patiently). So, three questions:
1. Anybody had a similar failure?
2. What clutch should I get (don't want to buy more than I need). Any guarantees? I need to get it fixed so I can redeem myself against that orange 3000GT VR4!
3. I'm itching to try a light flywheel (growing dislike for the unspirited free revving). Steel, or aluminum? Are they durable? Do I always need a counterweight with a lightweight flywheel (and why or why not)? In my ignorant state, I would get a 9.5 lbf flywheel from Pettit racing (lightweight, relatively cheap, and steel).
