Question what could have caused this

what would cause a rotor to do this, or does it look like mazda reman put it back togeather like this minus the cracked apex seal

ah, that doesn't look happy...I would say detonation.
but it could be a few other things.

Is that brand new, or has the engine been ran already? Looks too clean to be used. Not meaning used parts, just if the engine has been started yet.

looks like a cracked apex and a corner seal smashed into the rotor face

i bought the car and was told the motor had about 15k miles on it, car ran very very rich with supposedly stock maps on the power fc, motor popped one day on the highway under about 12psi, pulled it only to find the pic i showed you guys, there is also a nice gouge in the rotor housing about the same location where the chip is in the rotor

Awesome pic!

I'd say either another apex seal is missing or the engine might have eaten something...

I wouldn't doubt for a second that motor has pretty low miles on it.

That indentation is from a busted apex seal. Notice the shape.

how are the turbos? if the piece is not in the housing it made it out the exhaust. still looks like a corner seal

pulled the turbos, they are fine, called 200 places today, noone has rotors nor housings?!?!?!
