A significant hose leak definitely fits your symptoms: coolant level goes down in engine, and none is drawn back from the O-F tank.

I think the external seal leak scenario is relatively rare.

Yeah. Possible, but highly unlikely. Again a pressure test will show for certain where the leak is.
