MO Premature O2 sensor failure

Has anyone experienced premature O2 sensor failure when using wideband O2 sensor and Racing Beat Road Race Header? If you did, what have you done to resolve the issue? Did you use the Bung extender that is available from DIYAutotune and did it work?

Did you use any kind of heatsink or even header wrap? The sensor will overheat at about 800F, while EGTs can exceed 1600F. I use a 3x5" piece of 1/8" copper plate as a heatsink, and have only overheated my sensor 1 time since I've owned it.

How far was the sensor from the exhaust ports? Manufacturers recommend 36".
