kill it with BOOMSTICK!
the clutch pack is worn out in the LSD. buy a rebuild kit and spend a weekend doing that. Although they do grip better than the s5 LSD's, they also wear out faster.

its a LIMITED slip, not a no slip. the clutch on a lsd aonly engages when one wheel is spinning faster then the other, then the clutch grabs. with you just spinning it by hand there wont be enough friction to get the slip clutch to kick in

Get someone to try to turn the other tire the same direction, if it moves, that means your lsd is shot... At least thats what I was told. The S4 LSD dont last as long as the S5 ones, but they still last for 150-200k depending on the way you drive.

Yes, it has a LOT to do with driving. Generally for most 'standard' driving styles, they last about 120k or so, for those spirited autocrossers/etc, it's quite a bit less.
