Nope, you're not missing anything. The headrest speakers only came in leather seats factory.
I have cloth seats with headrest speakers in my S4 TurboVert only because I had my seats completely re-upholstered removing the leather and using cloth. Any Vert with headrest speakers and cloth would be only as a result of the same.

Reupholstered seats from another convertible is going to be the answer. My car has the leather package, but a previous owner had the seats reupholstered in cloth. They look factory, so I didn't even realize it had been re-done until I recently went through the '88 brochure & talked to other vert owners.

The 1988 convertible came standard with a cloth interior no head rest speakers or door speakers. Leather package with head rest speakers was an option in 1988. Assuming your interior is original, it could have come with cloth. Check the link and scroll down to the 1988 model spec sheet.
