This is just off the top of my head. There might be more but I think this is just aout everything. The prices are obviously an average cost. There are deals out there...
I would buy everything used except the seals for the tranny, clutch (and assembly), and have the flywheel turned.

Required:

Low mileage tranny- $200
Input and Output seals for your used tranny - $15
Flywheel / counterweight - $50
Shifter - $15 (With Bushings)
Shifter Surround with boot - $20
Clutch, pressure plate, throwout bearing, and pilot bearing- Anywhere from $150 to $350
Clutch Master Cylinder - $40
Clutch Master Cylinder Lines - $15
Slave Cylinder - $25
Pedal Assembly - $40
Mounting Brackets that need to be welded in - $45
Driveshaft - $45

Not Required but a good idea:
Manual Guage Cluster - $25
4:10 or higher rear end - $Varies a lot

Hope this helps,
Brian

It cost me just under $1k, but thats because i broke everything possible in my adventure. Good planning can get it around $500.
