my bov blew off.. literally

like the title says...
the blow off valve blew off the car.
anyone know any possible causes? i'm stumped.

('91 TII, emissions removed, 3' full exhaust, factory TID w/ cone filter)

car was right around 6k rpms when it happened. i think i saw a fire ball also, not sure though...

no i know i thought it was just so ironic. i lol'd at it as well.
it's an hks ssqv i think. i can't honestly remember right now. it looks like an old school model though. it's mounted between the turbo and tmic.

oh i had friends that the hks one could fly off too...thats why im running a greddy bov. people i talked to think its cause the hks is only held on by that clip and not really bolted onto the pipe

that's exactly what i thought. i guess i'm in the market for a new bov then :]
is it possible this was cause because i'm boosting too high? i'm on stock turbo btw.
