"Carbon lock" is actually rather rare. It happens when a large section of carbon dislodges itself from the face of the rotor, falls off and locks the engine from rotation. Another version occurs when carbon buildup on seals is so severe that they no longer move in their grooves.
The end result is the same in both cases: engine no longer rotates.
It can be cleared by dissolving the carbon, generally with carb cleaner, diesel fuel, etc.
