What causes flame-out?

Well as stated i was wondering what causes the RX to flame-out? I know from working on old VW's if you messed with the timing that would cause it. Is it the same on the RX's?

Running rich from the factory, exhaust heat, turbocharged, removed emissions

Running rich, no valves, completing the combustion stroke in the downpipe, etc...
