clutch grind

Quick question. I have a clutch problem, theres a grinding sound that happens when shifting into gear, especially in reverse. The problem has been getting progressively worse and now its beginning to grind when the clutch is pushed in if the shifter is in gear. It appears as though the clutch isnt fully releasing when i push the pedal. There is a new thow-out bearing in the vehicle. Also the clutch fluid reservoir was empty, at the begginning of the problem but i topped it up and it doesnt seem to be leaking at all anymore as its staying full. I didn't bleed the clutch lines after refilling the reservoir but the feel of the pedal is normal. Please provide any and all possibilities and tell me which ones would be more likely. thankyou.
