wondering if friend's engine is on its way out

Hey, my friend told me a few symptoms he's having with his FD and I wanted you guys' opinion. I have not seen any of this myself, so this is all second hand.

He says there is oil in his radiator. In a piston engine this would signify a cracked block. But he also says there is gas in the oil (at the filler neck, I assume) which creates a milky substance. What is this? And he says the car runs rough. Is the engine doomed? Oh yeah, it's got nearly 100K miles. I know, it's obvious. I'm just wondering how it's dying.

that milky substance at the top is normal in cold weather (like now). it's water in some sludgy state mixed with some oil and other assorted crap/debris. Gasoline smell in oil is also normal.

oil in radiator i don't think is normal.

if an apex seal is about to go or already gone, the car will idle low and like ****. check vacuum. You should read ~ -400-500 mm/Hg. if his car stalls when he pushes in the clutch after reving past say 2000rpm, that's a bad sign as well. if the car idles low that's a bad sign. normal idle is around 750-900 rpm. if he's seeing <= 500 or somewhere in the 600 range that's not good.
