What are the symptoms of an ailing airpump?

What are the symptoms of an ailing airpump?
aside from not passing smog - the obvious.

noise?

Yeah, I would say noise is really the only potential problem. It's a pretty simple device, really. If junk gets into the air pump, it could get REAL noisy.
