What causes 2nd turbo not turning on? for FD?

can anybody list any problems will causes the 2nd turbo not turing on????

Really, look through it. Gotta have the turbo control solenoid closed (but then opens at 4500 rpm), the wastegate closed, the charge control open and the charge relief closed.
