Time For a Rebuild?

Hi, i recently noticed that when ever i add oil I notice there's a mixture of coolant, oil (green sludge) and a strong stench of gasoline. I changed my oil yesterday and found no mixture from the oil pan. What can it be? Someone help me please, Thank you for your time.

Yeah under the filler cap, sorry for not stating that.

The coolant seems fine, I check it once a week and it never seems to be going down. But when ever I shut off the car I hear it boiling. Also when i check the oil filler passage it has a lot of moisture on the side of the pipe.
