I have a broken neutral switch (no continuity ever), and it will sometimes surge during warmup in certain temp ranges (PFC tuning issue), but stops when I put in the clutch. The signal (or lack thereof) reports to the ECU that it's in gear at all times. This actually started happening with the stock ECU, before I got the PFC.

I'd start by testing the neutral and clutch switches.

Most likely the clutch switch. I accidentally didn't plug mine in once, and was all WTF .

They're not too pricey new, but do a search - someone figured out how to fix one with a spring from a ballpoint pen .

Yeah, now that I think about it, only the clutch switch would make sense. If the neutral switch was bad, the condition would not change when you put it in gear. Sorry for the confusion.

The switch is above the clutch pedal lever. Push down the pedal and see if the shaft moves smoothly. If it sticks or does not return, replace it. If it seems fine, try testing the switch. See F-187 in the FSM.
