Parking brake adjustment at the "non-handle" end

How the heck do I do this? My parking brake adjuster has been cranked down all the way, and I still can't get a decent grip even with the handle all the way up. It feels like the cable has slipped somewhere - it used to be better than it is right now.

With no engine, I'm having a blast convincing the car to stay in one spot on the side of the road.

Alternately, is this a really good indication that I need new brake pads? Heh.

I have the same problem. I just replaced the rear pads and it still persists. It looks to me like the mechanism at the wheel end is going back pretty much as far as it can, but it's still not holding at all.

When I replaced the pads, I noticed that the disc has a tiny lip at the outter end, but there's plenty of disc material left, could that have something to do with the problem?
