Cold Start Idle Problems

Cold Start Idle Problems
I've searched and have not found an adequate answer yet as to my problem.
The Basics:
89 NA GTU MT
RB Header and Presilencer
Flowmaster Mufflers(previous owner had these installed)
Everything else is stock
The ecu is NOT throwing any codes. No codes stored.
The Problem:
Cold start in neutral, car revs to 3k for a second and then dies.
Cold start in gear, car revs to <2k for a second and then dies.
I can keep it running by giving it gas, but it backfires. I'd rather not have to start my 7 this way for the rest of its life.
I've quadruple checked that every vacuum hose is connected to the correct place and there are no vacuum leaks anywhere. I've never adjusted the TPS or the BAC.
What do I need to do?
I hope I was thorough at giving the details up front.

I could adjust them, but they have never been touched. I am certain there is not a vacuum leak.
Should I be looking at the water-thermosensor? I had removed the throttlebody at one time and all of the coolant had drained from the lines at the TB. Also, the BAC lines have been removed and coolant drained. Would those things have anything to do with my problem?
$50 to anyone who can get it right. But it has to work.
