Car dies out

Hey all. I have a 88 TII. Anybody have insight to the problems it's having? The car seems to not hold it's idle, subsequently dying out on me when driving it around in the streets. The catch is... it will idle fine when driving for the first 20-30 minutes or so, however, it seems like when the car gets warmed up after driving it for some time, it will began to die out on me. The idle just drops down and the engine goes off. Another problem I have that might be connected to this, when I'm accelerating reaching around 3,750-4000 RPM, the car will stutter and hesitate. Might be useful to know that I had it smogged and it was releasing lots of unburned fuel and had high carbon reading too. My friend said that the car might be miss firing too. Can these issues all be connected? Thank guys.
