Chances are they dont need to be replaced, but it cant hurt to pull them and re pack them with new grease.
That really helps them last.

The bearings in the front rotors are easy to replace. The inner ones can't be removed and reinstalled, so you have to replace them when you remove them. The outer ones though can be removed, repaked with grease, and reinstalled.
The whole set of bearings in the front (inner, outer and seal on both sides) isn't too bad a cost. Check Mazdatrix.
The rears are a PITA from what I've heard. You aparently need an axel puller and everything.
Don't bother doing your bearings until they NEED doing. You'll know when they need it, because they'll make noise when you turn in a given direction.
It's just a waste of money to do them beforehand. My car has 156k on it and the bearings are fine. My dodge shadow had 200k on it before the front ones started to go.
