What happened to this turbo?

Td60-1 p trim, the oil drain is good size , so I don't think that's the reason.
I let the car run for about 5 min after spirit friving.
I had a turbo blanket (I almost blame the hell out of this thing.)

I did not have an oil catch can at the moment. ( not enough crank case ventilation, maybe.?)
Oil feed it's -4AN. Inside diameter .1850 in.
I noticed the oil presure went really high so I desiderate to take the air filter and take a look and there it was shaft play that touched the comp housing.
I need input to prevent this to happen again to my new turbo.
