Throttle body with 2 throttle cable pullies?

Hey guys was just taking a look at my throttle body of my FD, never realized but looks like theres 2 spots that you can actually hook up the throttle cable? The pulley I use is obviously in a direct line with the cable, the other is just on the inside of that? I dont know a ton about rotaries but thought it was odd looking.... anyone know what it's for ?

Also noticed the inside pulley can be rotated independantly, while the main one with the cable attached doesnt move. Also when rotating the main pulley the inside pulley doesnt move.
