Unhappy keeps throwing a belt

My 93 RX7 keeps throwing the power steering belt. I have replaced the belts, tried replacing the pullys, added new wider pulleys, but it continues to keep throwing the belts. I'm at my wits end.. any suggestions?

If you've already ruled out a bad pulley then I would check their actual alignment.

yes twice.. set it to the right tension.the last time I tightened it more and it appears to have stayed on, but the outer edge is rubbing on the pulley and making a squeaky noice until the engine warms up.
I noticed that the steering wheel is slightly off center, could this have anything to do with the belt issue? the steering wheel was centered a few months ago but it appears to have gone off center again.

First off, the belt should be snug, not He-Man tight.

I had continual problems with power steering belt noise, then I flipped the idler pulley around. The idler pulley that you adjust belt tension with should have the flared lip towards the front of the car. There's some debate on what's the "real" way Mazda intended that to be set up, but flipping that pulley around solved all the problems on my car.
