can you get it out/off without pulling the primary fuel rail?

I couldn't. I pulled the whole rats nest out to get to it.

It seemed accessible with the tranny out, but I don't think that's less work than pulling the UIM and rats nest. Besides, you'll be able to change the TB coolant hose (if you have it) and do some critical vacuum hoses. The hoses that go down past the injectors to the block are IMO very important and often skipped when doing a hose job.
