No brake pressure + stalling still.....big vac. leak?

Well i think i have narrowed my stalling issue down to the vacuum for brakes. Before starting the car i have to pump brake pedal about 4 times to build pressure...while still holding pedal, i start car and the pedal drops to the floor (won't ever build pressure while car running no matter how many times pumped.)

I then held rpm at 2000, pushed brake pedal and it dropped to about 1500 until i let off. I can hear a slight "hissing" sound when brakes pressed to build up pressure. When the pressure builds, hissing goes away....All lines are connected. Few questions i have.

Two washers are supposed to be used on the banjo bolt going into manifold correct?
Anywhere else prone to leaking other than where the line connects to LIM?

If the "O" ring around the TB that mates with the plastic elbow is fairly flat in some spots, could that also be a culprit of stalling when not sealed completely?

It's an S5 n/a with a full "tune up" done, so i am sure it is not a fuel or spark issue..since it acts as if it wants to hold an idle before all of the vacuum gets released or something. Jumpered fuel pump test connectr also and started car but it still stalled. Any help or direction for further diagnosis is greatly appreciated.
