You either have a bad secondary injector, a bad connection at the secondary injector, no voltage getting to a secondary injector, or (if an 87 model with the injector resistor pack under the air filter) a bad injector resistor to one of the secondaries causing an open circuit. You could also have a break in the wiring harness or a bad injector driver in the ecu. If the ground clip on top of the rear rotorhousing under the throttlebody is loose it could also cause similar problems.

You're going to have to start checking voltages and resistances on the wires to/from the secondaries, and if that doesnt lead anywhere you'll have to splice in a noid lite and go for a drive to see if there is any injector pulse, since the secondaries are only fired under load.

IIRC the FSM has various "under load" testing done by venting the boost sensor to the atmosphere to simulate load - will that bring the secondaries online in the driveway?

I *think* that procedure only works for series 4 cars. Since the OP didnt specify, I felt it'd be best to go at it the other way.
