AFR Stuck at 16 Under Boost

So, this just started happening. when doing highway pulls as the boost hits maximum boost at 15 psi it starts breaking up [especially on initial non warmed up driving] and my PLX wide band shows 16 on the dot even after warmed up. nothing more nothing less. before under boost it was showing [9-10] . car runs fine normally, AFR shows normal readings. do you think my secondary injectors are crapping out?

First, stop driving like this. 
What is your ECU setup? Standalone? Stock?
If stock, check your FCD.
If standalone, turn on datalogging then carefully drive into this condition.

Change your fuel filter?
I took out my wideband to let a friend tune his car with it.
when I reinstalled the wideband, it said my fc was running lean. I adjusted fuel map and nothing happened. Turned out it was the fuel filter. New fuel filter and my afr’s went back to normal.

Faulty sensor, Fuel filter, fuel pump, fuel pressure, injectors
Having sit at dead on 16 like that especially with WI is extremely odd. I would swap out the sensor first and then do other things to troubleshoot

fuel filter is new. pressure is set to 45 psi. so first time since the car was tuned 7 years ago i hooked my laptop. i wish i know what the hell im looking at. lol.

DYNAMIC fuel pressure is whats important... STATIC pressure is cool but fuel pressure under load can tell a lot.
i would replace or even clean the wideband before going any further just to verify its not taken a dump

as far as i know, AEM is the only people that use special sensors. the rest, you can use a regular bosch sensor whether it be 4.2 or either of the 4.9 styles. so just get a regular bosch sensor presumably the latch style 4.2

so, i was mistaken. i have a AFX wideband. according to the manual the gauge only reads from 9-16 range. i installed the new sensor. calibrated it. increased the fpr a little, fuel filter is clean and flows good. still the same result. at this point i can only suspect secondary fuel injector. feels like the car doesn't pull like it used to...

tested MAP. it works too. only thing odd i found was when i unhook the air temp sensor datalogger shows 77 degrees and 90 degree when hooked up [even when it has 74 outside temp] . might not have anything to do with it.
