probably in the wiring, one wire signals the motor to go forward and another signals the reverse for it. maybe there is a bad relay?

It's called a ' fuseable link' driver side, under the hood, one is for the headlights only. replace the string looking fuse.

If they work OK using the light switch, then its likely the toggle on the dash - it has a spring with a ball bearing on top of it that can cause problems when the grease Mazda uses gets old, e.g. the ball bearing falls off the spring and you lose electric contact.

If they do not work with either the toggle or the light switch, then I would assume you could rule out the toggle switch and wiring and focus on an electrical problem in the engine bay.

I had the same problem, it was the 3 'string' type fuses on the driver side by the strut tower

This problem can also happen if you blow the fuse for the parking lights. That happened to me last week. But if your parking lights are coming on it should be the fussible link.

Definitely the fusible link. Stop troubleshooting other issues and replace the 'headlight' fusible link mounted to the driver-side shock tower.

You'll be glad you did and save a lot of time. This is DEFINITELY the cause. I know.
