what is the actual cause of this and what is the solutions... I need help asap..

Either your secondary injectors are getting sticky/plugged and you need to run some injector cleaner through there, or you have bad grounds on your engine and you should run some new ground wires (such from the negative battery terminal to the frame, and from the frame engine block) Sometimes that doesn't help, and you have to reground the ecu, that ones a little more difficult and I suggest you search for a thread on how to do that one.
