Anyone every had a side seal failure

Just a post for interest, has anyone ever had a side seal failure. If so - do you know what caused it? over-rev-kev? running lean? etc. etc.

i did except the guy who build my engine didn't give me a reason, check out my thread in rotary performance i got a few answers..."rotor bearings and side seal" check it out.
